Adult Day Program

Life Stride’s Adult Day Health (ADHP) program is designed to promote well-being and socialization for adults age 55 and over.

The program offers an alternative to costly facility-based care, providing older adults access to non-residential medical support and supervised therapeutic activities in an integrated community setting.

The Adult Day program is open:

Monday - Friday from 9:00 am to 1:00 pm.

Staff includes:

Center Director

Registered Nurse

Licensed Social Worker

Program Director

Activity and Support Staff

The ADHP operates under the Medicaid State Plan Home and Community-Based Services benefit. To qualify for ADHP services the Medicaid beneficiary will meet the following criteria:

Is age 55 and older

Is an adult with a chronic medical condition diagnosed by a physician

Has an income below 150% of the Federal Poverty Level

Is in receipt of an assessment determination authorizing and specifying level of need in accordance with Section 9709 of regulations
